This Online Healthcare Scheduling Request Form is the first step for scheduling visits in Santa Fe, Albuquerque, and/or TeleMedicine.

NOTE: We are NOT currently accepting any new Primary Care/Allopathic patients at this time. Only established Mountain Spirit patients who have already seen Dr. Melissa Gibbons, ND, and/or Lauren Zielinski, CNM, may schedule and receive Naturopathic Healthcare (or in the case of Lauren Zielinski, Women’s Health). Both Dr. Melissa Gibbons and Lauren Zielinski are considered SPECIALISTS.

*RETURNING PATIENT who has received care here since February, 2023? We prioritize your care needs & will be in touch with you soon to schedule!

*NEW PATIENT, who has Never been here for care (OR not since June, 2023) - Due to our Practitioners’ high demand: We are only able to take New Patient Requests through this Form. We’re typically able to respond to this Request in about 1 MONTH. From there: Look for Forms sent via our Patient Ally data system that reference Grasshopper Natural Medicine and/or Mountain Spirit, and complete them promptly. If you wish for us to bill any Insurance of any kind: You must provide your Drivers License, Insurance ID Card, and place a valid Credit Card on file prior to scheduling any appointments here. Many clinics as successful as Mountain Spirit have discontinued taking New Patients. We’re willing and able to do so — but only so long as New Patients complete all Forms and required items in advance of scheduling their first appointment here. To keep this process running smoothly and efficiently, we are not taking or responding to any phone calls or messages for New Patient requests.
